2017-18 Free & Reduced Price School Meals Household Application and Verification Forms

The 2017-18 Free & Reduced Price School Meals Household Application and Verification forms are used for determining student/household eligibility for school meals, notifying households of benefits, conducting verification, sharing Information, and notification of Direct Certification.

F&R Priced Application with Verification Forms

Prepare and submit the SFA F&R Application and letter templates on SFA letterhead in the completed state as if it is ready to be distributed to households. 

Email the SFA Free and Reduced Priced School Meals Household Application packet for review and approval to:

*NOTE*SFAs must submit their F&R App and Verification forms to the state agency for approval prior to distribution to households.

Web-based Application Checklist SFAs must obtain written approval from the State Agency and their local Superintendent or Board of Education in order to use web-based meal applications. The attached checklist must be reviewed, completed and signed by the School Nutrition Administrator. Once the checklist is signed and the requested documentation (i.e. procedures, Board Attorney’s signature) is attached, submit to your Operations and Management Consultants for approval.

**IMPORTANT NOTES** 

  • SFAs with year round schools should submit renewal documents and information as soon as possible in order to complete the entire agreement renewal process by the deadline, June 1, 2017
  • Submitting the a F&R School Meals Household Application for approval is not applicable to SFAs that have implemented the Community Eligibility Provision (CEP) district wide.
